Event Planning Form 
Use as a tool to plan ALL EVENTS.

Press Release Template 
Make a copy and save in the appropriate event folder. If you can't find a folder for your event on our google drive, contact Stephanie Schleicher at [email protected].

Hold Harmless Form
One form for every artist/collaborator, once every 2 years, for ALL EVENTS.
Download, fill out, attempt to print, then choose "Save as PDF." Upload the PDF to the appropriate folder.  Physical copies may also be available at the event.

Budget Form (for ALL EVENTS)
Make a copy and place in the folder below.  Approximate expenses.   

Fire Safety Template (For ALL THEATER EVENTS) 
Make a copy, attempt to print, then choose "Save as PDF." Upload to the appropriate folder below. You will have to identify a "Fire Watch Person" as well as some key volunteers.  FSEP must be e-mailed to the town officials before the event and also printed and placed in the ticket booth.

Art Exhibition Agreement (for ART SHOWS)
Download, fill out, attempt to print, then choose "Save as PDF." Upload the PDF to the appropriate folder.

Closing Checklist (for ALL EVENTS)
Fill out physical copy at the Studio leave on office door.  If you forgot, use this form.

Federal W-9 Form (needed from PERSONS COMPENSATED $600+ IN YEAR)
Fill out physical copy at the Studio leave on office door.  If you forgot, use this form.
