Event Planning Form (use as a tool to plan ALL EVENTS with Events Coordinator)

Hold Harmless Form (one form for every artist/collaborator, once every 2 years, for ALL EVENTS)
Download, fill out, attempt to print, then choose "Save as PDF." Upload the PDF to the appropriate folder below.  (Physical copies may also be available at the event.)

Budget Form (for ALL EVENTS)
Make a copy of the form and place in the folder below.  Approximate expenses. (You can also make a simple list of expected income and expenses and upload that if you prefer.)

Fire Safety Template (For ALL THEATER EVENTS)
Download, fill out, attempt to print, then choose "Save as PDF." Upload to the appropriate folder below. You will have to identify a "Fire Watch Person" as well as some key volunteers. 

Art Exhibition Agreement (for ART SHOWS)
Download, fill out, attempt to print, then choose "Save as PDF." Upload the PDF to the appropriate folder below.

Closing Checklist (for ALL EVENTS)
Fill out physical copy at the Studio leave on office door.  If you forgot, use this form.

Federal W-9 Form (needed from PERSONS COMPENSATED $600+ IN YEAR)
